The Philippine Society of Nephrology recognizes that kidney disease does not choose demographics- with a considerable number of children and adolescents included in the spectrum. However, as the young patients progress on to adulthood, their needs, physical and psychological evolve as well.
Thus, in 2003 the Lipat Kalinga Wellness Program (which roughly translates to Transfer of Care), was conceived as a nod to this predicament. The Program provides guidelines for pediatric and adult nephrologists to assist and ease in the transition to adult renal medicine.
